the game is now accessible with the use of mods and the topic is being updated with links to reflect the situation of the game, i'll past the description of the game bellow, plus the link to reddit's original topic that got attention from the community:
for those who don't want to install steam, looks like gog has the game to download in an accessible and drm free formate. there's a complete accessible package to use with it, it is being maintaned and updated by the user @firefly82.

a user posted on reddit that he made a mod so his girl who's completely blind could play it at the start of quarantine. she spent more than 450 hours playing the game and it's fully accessible with this mod, it's pretty self explanatory with the readme, but if you have any questions, just ask and i'll do my best to help!

@pitermach developed a program to interact with text the spire which takes the mod to the next level, it works even with sapi, if you don't have a screen reader running.
you can interact with the mod by using alt and arrow keys, alt home and end to go to the start and end of the buffer, or alt c to copy stuff to the keyboard. this way you can simply focus the prompt window and play without switching windows or interruptions.

Installation:
Note: This guide is for the steam version of the game.
Step 1:
If you have not yet, install Steam, create an account, and install Java.
Step 2:
Download Slay the Spire from the Steam webpage. You do not need to do so through the Steam client which is harder for screen readers. Subscribe to both BaseMod and ModTheSpire.
Step 3:
Find the Slay the Spire directory. In Windows the directory path should be similar to C/Program Files x 86/Steam/steamapps/common/SlayTheSpire.
Step 4:
Create a shortcut of the file mts-launder.jar on your desktop. This is how you will start the game.
Step 5:
Double click mts-launder to launch the mod. The first time you will need to select the mods. There is a select all option on the Mod the Spire window that will show up. Afterwards they will be auto selected. Hit enter to start the game. One more step.
Step 6:
You will need to set a save file name if you don't have a save file yet. Output should mention this as well. Go to the main Slay the Spire window, type your name, and hit enter. You are now free to play the game. Type help in the prompt to read about controls.

berenion, the game is currently playable, even more with the map mod to announce the squares, but as i said it's time consuming to use ocr every second.
but i'll do my best to get there, the aim is for screen reader support. there are comments saying that they would buy the game again if the developers add the feature.
only time will tell!

Those are good news. Slay the Spire is one of the games inspiring myTrueSound in the development of Kilta. Hopefully one day, accessibility is the standard in all new games. So yeah, let's keep fighting for it.

I'm still on win 7, so I don't have access to Win 10 OCR, but I'm working on changing that.
Would the map mod work with the GOG version too? I like to support them instead of Steam whenever I can.

Well the readme say that a blind person has played over 250 hours of the game using it. I guess OP should try it out and report back as I do not own the actual game to test it.

thanks for beating me to it and posting the link to this ultra super awesome mod, i can confirm it works and can now safely recommend everyone to smash the buy button and get the game real fast!
guys, everything is being read for us! in an accessible and easy formate!
you can play the game only with the keyboard, you can check stats, powers, buffs, potions, the map, I mean the whole game.
I know it sounds like I'm exaggerating but I'm glad I'm not!
who did this mod really deserves some donations!!!
for the user asking if it works with the gog version, it might work because it supports mods as well, the process of installing them might be a bit different to steam though, I'll must research in order to help you!
with the steam version it's just a matter of following the readme and you'll get things running fast.
the accessibility mod opens lots of windows, the only interest to you when the game launches will be the prompt and output window, so be sure to put both windows close to each other, you will launch commands to read what's written in the other windows only with the output, otherwise things will get confusing.
be sure to check the help command in the prompt window, things will be self explanatory and if you have questions come back here. I'll edit the first post with the mod info

as for steam, you may need a hand creating an account, but after that there should in theory be no issues.   I've been using it for years without any.  even turned off steam guard which helps.

a few more updates guys: the only screen which the mod is not currently reading is the outcome of events, which you can still ocr the main screen to understand. this will be fixed in a future update and is not a big deal.

For anyone looking to buy this game, which I'm going to do myself in a bit, I almost pulled the trigger on the Steam version but decided to see if there's a price difference between it and Gog... And it looks like the Gog version is currently discounted quite a bit.

just be careful with the gog version due to the instalation of mods being a bit different. with steam workshop you can get mod the spire and the base mod from the workshop by simply subscribing to them and it's guaranteed to work. i'm really not sure about gog.

Unfortunately it seams like the mods are tied to the Steam version. There are only 3 mods that I found on Nexus mods, so the Steam workshop is the only way you can get access to 99% of mods.

Okay, can anyone tell how to play this game? I start out as an ironclad, starting works okay. I use the map command, then i use choices command to read the choices which are available i presume, i write 1 in the window, then i hear cards drawing and i have no idea what the hell i should do.

hey dan! you can read everything with the appropriate command. for example to read the hand type hand in the prompt and use the output window to check the new info. i suggest hiding the other windows and check the help frequently.
. read choices frequently outside combat, every command is needed and useful.
the attack cards must have the enemy number, even if there's only one enemy on screen you'll have to type play card number enemy number.
there's help for every command. most of them accept extra parameters. don't be afraid to explore and read help multiple times to get used to it
arnold, check your hand and all the cards with the hand card number parameter. type player and monster, that's enough to get you started
