It's actually really simple. I believe most hosting providers will do the hole setup process for you, so you'll just need to connect to your server with rdp and type narrator in the run dialog. From there you should be able to get nvda or whatever screen reader you use installed. Also, depending on who you're with, they may or may not be able to help you get nvda / nvda remote setup and working if narrator turns out to be a no go. 1and1 are good examples of hosting providers who'll understand about screen readers and the like, also wiztech, though they're currently under going some server issues they're working through.
